What Are Perimenopause and Menopause?
Perimenopause is the transitional phase leading up to menopause, often beginning in a woman’s late 30s or 40s. During this time, estrogen and progesterone levels fluctuate unpredictably, leading to physical, emotional, and metabolic changes.
Menopause is defined as 12 consecutive months without a menstrual period and typically occurs around age 51. Hormone levels stabilize at lower levels, but symptoms — and health risks — can persist for years without proper management.

- Reduces risk of osteoporosis and fractures
- Favorable cardiovascular profile when started in appropriate patients under age 60 or within 10 years of menopause
Progesterone
Helpful for:
- Any woman with a uterus using systemic estrogen
- Protecting the uterine lining from estrogen-induced hyperplasia
- Improving sleep quality
- Reducing anxiety and mood instability in some patients
Forms:
- Oral micronized progesterone
- Combination estrogen-progestin therapy
- Intrauterine progesterone devices (off-label in select cases)
Evidence:
- Required for safety when estrogen is used in women with a uterus
- Micronized progesterone is associated with better sleep and fewer metabolic side effects than synthetic progestins
Testosterone for Women
Helpful for:
- Low libido
- Persistent fatigue or low motivation (after other causes are excluded)
- Reduced sense of well-being in select patients
Forms:
- Low-dose transdermal preparations
Evidence:
- Strong evidence supports testosterone therapy for treatment of low libido in postmenopausal women
